Survey Request Checklist

CTLT occaisionally does online surveys for faculty and staff. In order to better utilize CTLT's resources, we require that you provide the following information:

• Proof of IRB Approval
• At least 4-6 Weeks notice before survey goes 'live'
• Survey questions must be submitted to CTLT in electronic format (.doc)
• Client signs off on and is responsible for final testing

CTLT provides the survey tool, but no data tabulations. Survey results can be saved in Microsoft Access database format, or sent via e-mail.
