Dear Student:
Primary health care is offered at the Health Services, on an outpatient basis, from Monday through Friday during daytime hours. Health education is an important part of our service and health issues programs are offered at various sites on campus for all students.
The Health History & Immunization form is confidential and should be returned to Health Services before classes begin. If you have previously completed the form you do not have to return it unless there has been a change in your health history or your immunization record.
There has been a great deal of media attention concerning bacterial meningitis. This has led to the perception that there are more cases than usual in the United States . However the incidence of bacterial meningitis is very low. I encourage you to discuss the disease and the need for the meningitis immunization with your health care provider. Murray State University Health Services does not have an immunization program and we refer to local providers, such as the Public Health Department or physicians, for immunizations.
The American College Health Association has recommended immunization for those infectious diseases that are listed on the immunization form. The United States Centers for Disease Control and Prevention recommends that you be informed of the availability of the meningitis vaccine but has not required mass immunizations. The State of Kentucky requires that you be informed of the availability of the meningitis and hepatitis B immunizations.
Once again, I urge you to discuss all immunizations with your health care provider so that you can make an informed and appropriate decision about what is best for you. Please complete the health history and immunization form and if you do need any of the vaccines, have them administered before you arrive on campus.
