Information and updates on H1N1 Influenza (Swine Flu):
H1N1 influenza or what has been called swine flu is a respiratory disease that is caused by the influenza A virus. H1N1 flu can infect and be passed among humans. It is important to know what the symptoms are and important to practice preventive measures that help prevent the spread of not only swine flu but many other viral and bacterial infections. There is no need for panic; so far, cases in the United States have been mild.

We are very interested in providing the best confidential health care possible for students while they are at Murray State. Often this
requires having knowledge of past health history, current immunization status and family history. We now have the form
to complete available for you in .pdf format to print and complete. We would like to encourage you to take the time to
complete the Health History. This form may be mailed to Health Services [the address at the bottom of this page is correct - the one on the form is not].
The Risk Appraisal (also in .pdf format) should be completed by the student and brought to Health Services. The purpose of the Risk Appraisal is to give the student insight into areas of his/her wellness that may need improvement as well as potential threats to wellness. This is an excellent way for the staff to partner with students on an individual basis to make some significant changes in their health.
