Please contact us if you would like to speak with us directly. General Information tells you more about our service - mission, clinic hours, what we offer, and who we are. You will also find links to listings of physicians and dentists in the Murray area. We are very interested in providing the best confidential health care possible for the Murray State Community. Often this requires having knowledge of past health history, current immunization status and family history. We now have the form to complete available for you in .pdf format to print and complete (preferably prior to coming in to Health Services as this will lessen your time in the clinic). We would like to encourage you to take the time to complete the appropriate Health History. This form may be mailed to Health Services or brought in for your first visit. The Risk Appraisal (also in .pdf format) is an option for the Murray State student. If he or she chooses to complete the form, it may be mailed or brought to Health Services. The purpose of the Risk Appraisal is to give the student insight into areas of his/her wellness that may need improvement as well as potential threats to wellness. This is an excellent way for the staff to partner with students on an individual basis to make some significant changes in their health. Faculty and Staff will have options available to them regarding a Risk Appraisal. They should consult with Human Resources and the Wellness Committee as to how to complete this through the Anthem web site. Intelihealth.com has excellent resources available for everyone - particularly women. The Breast Self Exam Instruction Video is a good example of their available resources. Health Clips provides you with informational articles on a variety of subjects. Pay particular attention to the information on meningitis and meningitis vaccine. If you are a smoker and want the benefits of becoming a non-smoker, there is information on smoking cessation. Health Services and its nursing staff members are proud members of the American College Health. This organization provides us with up-to-date college health information. This in turn helps us provide excellent care for the students at Murray State. |
