Survey Request Information and Checklist

CTLT currently provides online survey support for faculty and staff using an annual license of Survey Monkey. Because Survey Monkey provides only one account, all survey creation and deployment using the CTLT license of Survey Monkey must be managed by CTLT.

In order to use CTLT's resources, we require that you provide the following information:

  • Proof of IRB Approval, or statement of exemption. (For IRB purposes, no personally identifying information will be asked or captured through the survey, including IP addresses.)
  • At least 4-6 weeks' notice before survey goes 'live'
  • FINALIZED survey questions, submitted to CTLT in electronic format (.doc)
  • Timeframe (or other criteria) for survey to be active. We will set the survey to stop collecting entries when your criteria has been met - this may be a date or maximum number of responses.
  • Messages that you would like the participants to see when they submit the survey, when the survey is closed (no longer accepting entries), or when the visitor does not meet criteria to participate in the survey.
  • Password for viewing and downloading the results.
  • You (the client) are responsible for final testing. When you have completed your testing, contact CTLT to remove the test entries.
  • CTLT will provide a password-protected link for you to view and download the results from Survey Monkey. You may filter and sort the results as needed. CTLT does not offer data tabulation support.
