3rd Year AHEC Rotations

Review the list of preceptors that have recently taken students.

Complete the AHEC Rotation Request Form with up to 4 choicess.

Make note of any special circumstances that are impacting your choice (I used to work there, I shadowed there, I received a recommendation from another student, I have family I can stay with, etc.) - the more that's known up front about the reason for your request will help in making placement decisions.


Complete the W-9 form for your housing, travel, and meal reimbursements. U of L reimbursements go directly into your student account.

AHEC Rotation Request Form and a W9 Form (for reimbursements) must be completed and submitted to Anne Loop by March 9th, 2010. If you have any questions regarding the process, Anne can be reached at 502-852-5314 (Med Center 1 Building, Suite 270).

The Decision Process: Once your rotation requests are in and reviewed, Anne will forward the requests to the AHEC office. We contact the physician with the request, confirm placements back to U of L and then Anne will notify you to complete a Fact Sheet. The form will then be forwarded to our office to be used as a reminder to physicians, hospitals, housing sites (if applicable) that you will be assigned to during the rotation.

You will be sent a reminder at least a month prior to your rotation with the specifics of contacting your preceptor, completing orientation, directions and anything else that will streamline the process once you arrive on site. The more administrative stuff that can be done before you arrive will give you more time with your physician learning.

4th Year AHEC Rotations

4th year AHEC Rotations may be selected based upon your area of interest. The process for these requests vary by department, so contact the U of L student coordinator in the department associated with the specialty you are interested in.

Contact the department coordinator with your request.

Once your rotation request has been approved and confirmed (either by dept.or AHEC), Complete a Fact Sheet and submit it to the department student coordinator.
