Community Analysis/Target Assessment
With your small group choose one (1) of the following and perform a
community/group analysis. Confirm plans with instructor prior to initiating.

Community: Perform a community analysis on a county in western Kentucky.
Using Dignan and Carr, pgs. 37 – 45, Kentucky Vital Statistics, BRFSS,
and HP2000 include age, racial and gender composition; number of households;
gross family income/poverty levels; marital status; education level, morbidity;
mortality; birth rates; infant and fetal mortality rates; years of life
lost; and risk behaviors for the most recent years. Compare to Purchase
District and State. You may use the data collection sheets provided or
design your own. Identify trends and high-risk groups. What is the next
step? (i.e. targeted group assessment, etc.)

Focus Group: Choose a small group (6-12), get permission, plan and
perform a focus group on a topic of concern to the group. Use the instructions
from Dignan and Carr, page 28 and handouts in class packet. Include demographics
of group, statement of problem or area of concern, plan for leading the
discussion, summary of opinions, relate results to the general population
and finally, describe the next step (i.e. more intensive needs assessment,
community survey, etc.).

Employee Wellness Survey: Choose a business (10 – 20 employees),
get permission and perform an Employee Wellness Survey. Make sure to provide
for confidentiality of participants and obtain signed consents from employer
and employees. Your group may choose an instrument or use the PAR-Q. Provide
individual reports for the employees and a group report to be turned into
the instructor (not provided to employer). Make recommendations concerning
appropriate interventions, based on employee needs.

School Site: Choose a school, get permission and perform an assessment
of four (4) of the 8 areas of Comprehensive School Health (Kane, 1993).
Use the worksheets found in chapter 2. Summarize the findings and make
recommendations for improvement.

Health Care System: See Dignan and Carr pages 45 – 50. Collect data
identified on these pages from one of these communities: McCracken, Graves,
Calloway or Marshall County. Contact the area hospital and request discharge
information and home health visits for a one-year period. Identify gaps
in service, leading diagnosis used for inpatient and outpatient services,
and potential resources or solutions.
Type each person’s name at the top of your report.
Criteria for grading: equal participation by all group
members; inclusion of specific assessment instrument and target population;
summary and analysis of results; next step recommendations; and errorless
grammar, typing and spelling.
